Frequently Asked Questions

  • If you are not a Professional Referrer and would like to refer yourself or someone you know for Crossroads support, please use our dedicated Self-Referral Form instead.

  • Professional referrals are key to supporting older and disabled people in the local community.

    While Crossroads will take every reasonable step to contact and support the individual(s) you refer, here are some common reasons that unfortunately delay or prevent provision of services to the named individuals:

    • Uncertainty around the individual’s capacity to consent and no authorised professional details provided

    • Missing, incorrect, or outdated contact details

    • Contact details provided for a third party without permission to use them

    • No named professional to liaise with where risk or vulnerability is indicated

    • Insufficient information to confirm eligibility for services

    • Unclear funding source

    • Conflicting or unclear information that requires clarification before contact can be made

Professional Referral Form:

This form is split into 4 sections: Services, Your details, Main Carer details and Cared For (Client) details.

Please use the textboxes to clearly indicate whether your Service User is the Main Carer or the Cared For.

We appreciate that community referral forms can be lengthy, so we’ve simplified this form with multiple choice answers and tickboxes to make your referral process as smooth as possible. The mandatory fields are essential for us to build a clear picture of the people we support, quickly identify eligibility and help to triage individuals to the most appropriate service, ensuring we provide award-winning, tailored care right from initial contact.
